So in Dreamweaver cs5 when i want to use a MySQL Database

i need to know how to add and use a MySQL database from my package provided by Vlexo hosts

in the < MySQL Connection > window that opens in dreamweaver i need to fill the following:

Connection Name = ""
MySQL Server = ""
Username = ""
Password = ""
Database = "" ( how to create a database in my package provided by Vlexo free host )
